Washer would only be to time the holes on top

How are the holes supposed to be positioned ? Right now the wide holes are on top. And where do I get a washer if I need one?

Wide holes to sides, there should be 3 or 4 round holes depending if its a 3 orv4 port, those will be on top.

I'll look for the link for 11/16 shim washers.
